The Children with Disabilities Team

What  the service is:

The Children with Disabilities Team (CWDT), placed within the All Age Disability Service, is a specialist Social Work team providing support, advice, assessments and access to specialist services to children and young people with disabilities and their parents/carers resident within the London Borough of Hillingdon. 

Who the service is for:

Eligibility criteria:

  • Children or Young people with Disabilities aged from 0-25
  • Resident within the London Borough of Hillingdon
  • Has a substantive and severe learning and/or physical  disability and/or
  • Has complex health needs and/or
  • Has a severe autistic spectrum disorder

When to use the Service:

Typically the child or young person meeting the above criteria will require a range of services to promote or safeguard their health, development and/or well-being.  Interventions/support identified is outcome focused and aims to minimise the impact of the disability.  Support with adaptations, challenging behaviour and short breaks respite are some of the interventions that may be offered following an assessment to determine the needs of the child and their parent/carer. 

The Children with Disabilities Team works in partnership with schools, health and other professionals to ensure the best possible outcome for the child/young person and their family/carers.
